The ideal candidate holds a CAPTE-accredited degree, current state licensure, CPR certification, and a valid driver's license to travel to different client sites.

Qualifications

• CAPTE-accredited PT program required.
• Active state license to practice PT.
• CPR certification required.
• Licensed driver with ability to travel to locations.

Responsabilités

• Provides patient care and documents treatment per physician orders and agency policy.
• Performs patient evaluations and selects diagnostic materials.
• Informs physician of assessment results and recommended interventions.
• Develops and documents treatment plans and establishes priorities.
• Provides organized patient treatment, including education and counseling.
• Presents patient information to physicians and informs team of patient status.
• Delivers treatment within agency and payer parameters.
• Establishes measurable goals in collaboration with team and patients.
• Meets productivity standards and uses work hours efficiently.
