Job summary

Top Echelon Software, LLC is seeking a dedicated Physical Therapist to join our home health team in the Denver area. This full-time role focuses on delivering personalized patient care in patients' homes, while documenting progress and coordinating with physicians.

The ideal candidate has a CAPTE-accredited PT degree, a valid state license, CPR certification, and the ability to travel to various locations. A patient-centered approach and teamwork are essential.

Qualifications

• Minimum nine months experience as a physical therapist.
• Preferred community/home health experience.

Responsibilities

• Provides patient care and documents treatment in accordance with physician orders, agency policy and patient needs.
• Performs patient evaluations and selects appropriate diagnostic material to assess patient care.
• Informs physician of assessment results and recommended interventions before initiating care.
• Develops and documents treatment plans with chosen interventions and priorities.
• Delivers organized patient treatment, including education and counseling.
• Shares pertinent patient information with physicians and keeps team informed of status and plan.
• Provides treatment within agency and payer parameters.
• Sets measurable goals in collaboration with team and patients.
• Meets productivity standards and uses work hours efficiently.
