Keystone Distinction Group is seeking a dedicated Physical Therapist to provide in-home health care across Colorado. This full-time role focuses on delivering high-quality, patient-centered therapy directly in patients' homes.

You will assess, plan, and implement rehab programs, educate families, and collaborate with a multidisciplinary team to ensure comprehensive care while documenting progress and maintaining compliance with standards.

Qualifications

• Minimum nine months experience as a physical therapist.
• Preferred community/home health experience.

• Maintain meticulous patient records and documentation, complying with regulatory standards.

Qualifications:

• Minimum nine months experience as a physical therapist
• Preferred community/home health experience

Required Certifications and Licensures:

• Master's degree in physical therapy approved by CAPTE accredited physical therapist education program
• Currently licensed to practice as a physical therapist within the state of agency operation and compliant with accepted professional standards and practices
• Must possess and maintain valid CPR certification while employed in a clinical role
• Must be a licensed driver who can travel to all business locations
