Client Growth Resources, Inc is seeking a dedicated Physical Therapist to join our home health team in the Denver area. This full-time role focuses on delivering patient-centered care in patients' homes and documenting progress in accordance with physician orders and agency policies.

You’ll perform evaluations, develop and adjust treatment plans, educate patients, and communicate with physicians. Eligibility requires CAPTE-accredited Master’s, current license to practice, CPR, and a valid

Qualifications

• Master’s degree in Physical Therapy from a CAPTE-accredited program.
• Current license to practice as a physical therapist in the operating state.
• CPR certification must be valid during employment.

Responsabilités

• Provides patient care and documents treatment per physician orders and agency policy.
• Performs patient evaluations and selects diagnostic material for care assessment.
• Notifies physician of assessment results and recommended interventions.
• Develops and documents treatment plans with priorities and intervention methods.
• Educates and counsels patients for organized, efficient treatment.
• Shares patient status and treatment plan with physicians and team members.
