Job summary

VitalCaring seeks a Home Health Physical Therapist to deliver patient-centered care in patients’ homes across Florida. You will assess mobility, design individualized plans, and work with physicians and care teams to enable safe, independent living.

You’ll supervise PTAs where permitted, educate patients and families, and contribute to discharge planning. A state PT license, DPT or Master’s degree, CPR, and 1–2 years of home health experience are preferred. A reliable vehicle is required.

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or Master’s)
• Current PT license in the state of practice
• Minimum 1–2 years of experience (home health preferred)
• Strong clinical assessment and critical thinking skills
• Excellent communication and patient education abilities
• Current CPR certification
• Valid driver’s license, reliable transportation, and auto insurance

Responsibilities

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential
• Develop and implement individualized care plans in collaboration with physicians and the care team
• Provide hands-on therapy to improve mobility, strength, balance, and independence
• Educate patients and caregivers on treatment plans, exercises, and safety strategies
• Monitor patient progress and adjust treatment plans as needed
• Support safe transitions of care and contribute to discharge planning
• Identify additional patient needs and collaborate with interdisciplinary teams
• Partner closely with nurses, therapists, and physicians to ensure coordinated care
• Document patient care and progress accurately and timely within the EMR
• Participate in case conferences and team meetings
• Supervise Physical Therapist Assistants (PTAs) in accordance with state guidelines
• Provide guidance and education to caregivers and support staff
