Job summary

Family Life care seeks dedicated Physical Therapists to join our home health team in Florida. You will manage patient care across various assignments, from case management to long-term recovery, with a schedule that fits your needs.

Duties include assessments, treatment per plan, and collaboration with care managers; you will use EMR and EVV systems and uphold Medicare/Medicaid guidelines while delivering compassionate, professional care to patients in their homes.

Qualifications

• Current Florida Physical Therapy license.
• Current CPR certification.
• Driver's license and vehicle insurance.
• TB test with negative results.
• Ability to pass a Level II background check.

Responsibilities

• Assess patients and coordinate care with physicians and care managers.
• Provide home health physical therapy per plan of treatment and policies.
• Deliver care in a courteous, professional manner to maximize outcomes.
• Complete clinical assessments, care plans, and resumption of care per guidelines, deadline-driven.
• Train, coach, and direct clinical staff in clinical duties.
• Ensure compliance with state and federal laws and home health regulations.
