Job summary

VitalCaring Group is seeking a Home Health Physical Therapist to deliver patient-centered care in patients’ homes across the state. You will assess mobility, develop tailored plans in collaboration with a medical team, and provide hands-on therapy to restore function and independence.

You will educate patients and caregivers, monitor progress, and help coordinate safe transitions of care. APTA-aligned practice, CPR, and state licensure are required, with opportunities for growth and leadership

Qualifications

• Graduate from an accredited Physical Therapy program (DPT or Master’s).
• Current PT license in the state of practice.
• Minimum 1–2 years of home health experience preferred.
• CPR certification is required.

Responsibilities

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential.
• Develop and implement individualized care plans with physicians and the care team.
• Provide hands‑on therapy to improve mobility, strength, balance, and independence.
• Educate patients and caregivers on treatment plans, exercises, and safety strategies.
• Monitor patient progress and adjust treatment plans as needed.
• Support safe transitions of care and contribute to discharge planning.
• Identify patient needs and collaborate with interdisciplinary teams.
• Document patient care and progress accurately in the EMR.
• Participate in case conferences and team meetings.
• Supervise Physical Therapist Assistants (PTAs) per state guidelines.
