Qualifications

• Must be a graduate of a Physical Therapy program approved by the AMA/APTA.
• Licensed or registered by the state without restrictions.
• One year of home health experience required.
• Knowledge of Medicare and Medicaid guidelines.
• Must have a valid driver’s license and reliable transportation.
• Current CPR certification required.
• Excellent verbal and written communication skills.
• Basic computer skills (e.g., Microsoft Windows).

Responsibilities

• Evaluate patients and develop individualized treatment plans based on physician orders and clinical findings.
• Establish functional goals to improve mobility, strength, balance, endurance, and quality of life.
• Provide skilled physical therapy interventions per the plan of care.
• Assess progress and modify treatment plans to achieve optimal outcomes.
• Educate patients, families, and caregivers on exercises, fall prevention, safety techniques, and equipment use.
• Complete necessary clinical documentation accurately and on time.
• Coordinate care with physicians, nurses, PTAs and other health professionals.
• Supervise and direct PTAs in line with state regulations.
• Participate in discharge planning and recommendations for continued care and services.
• Provide home health and outpatient therapy services as assigned.
