Job summary

Discovery at Home in Florida is seeking a licensed Occupational Therapist to deliver skilled home health and outpatient services. You will evaluate patients, develop and implement individualized treatment plans, monitor progress, and educate patients and caregivers to improve mobility and prevent injuries.

Responsibilities include documentation, collaboration with healthcare professionals, and supervisory visits when needed.

Qualifications

• OT degree from an accredited program.
• Licensed or registered by the applicable State without restrictions.
• One year of home health experience.
• Knowledge of Medicare and Medicaid guidelines.
• Valid driver's license and reliable transportation.
• Current CPR certification.
• Excellent verbal and written communication skills.
• Basic computer skills; ability to use Microsoft Windows.

Responsibilities

• Conduct initial evaluations to assess patients' needs and develop individualized treatment plans
• Develop and implement appropriate treatment plans based on patient needs and goals
• Monitor patient progress and adjust treatment plans as necessary
• Educate patients and caregivers on proper techniques to improve mobility, reduce pain, and prevent injuries
• Maintain accurate and timely documentation of patient progress
• Collaborate with other healthcare professionals to ensure patients receive comprehensive care
• Provide home health and/or outpatient services as needed, including supervisory visits, to ensure optimal outcomes
