Job summary

Discovery at Home is seeking a licensed Physical Therapist for full-time home health services in Florida. You will perform comprehensive client assessments, develop treatment plans with physicians, and implement therapies to improve pain relief and functional outcomes.

Responsibilities include supervising PT assistants, educating families, and ensuring accurate OASIS documentation. A valid FL license, CPR, and home health experience are required.

Qualifications

• Graduate of a Physical Therapy program approved by the AMA Council and the APTA.
• Licensed or registered in the applicable state without restrictions.
• One year of home health experience required.
• Knowledge of Medicare and Medicaid guidelines.
• Valid driver’s license and reliable transportation.
• Current CPR certification.
• Excellent verbal and written communication skills.
• Basic computer skills; ability to use Microsoft Windows.

Responsibilities

• Provides initial and ongoing comprehensive assessments of the client’s needs (OASIS).
• Establishes a treatment plan in consultation with the physician and revises it as needed.
• Treats clients to relieve pain and restore function using physical means.
• Teaches, supervises, and counsels the family and client in the PT program.
• Instructs client in care and use of mobility devices.
• Supervises PT Assistants and paraprofessionals at least monthly.
• Participates in case conferences and staff meetings.
• Completes and submits OASIS assessments and related documentation.
