Job summary

Discovery at Home is seeking a licensed Physical Therapy Assistant to support patient care in home health and outpatient settings in Florida. You will assist the Physical Therapist, help patients with ADLs, and educate families to promote mobility and safety.

Qualifications

• Graduate of an accredited Physical Therapy Assistant program
• Must be licensed by the State as a Physical Therapy Assistant
• One year of home health experience preferred
• Knowledge of Medicare and Medicaid guidelines
• Valid driver’s license and reliable transportation
• Current CPR certification
• Excellent verbal and written communication skills
• Basic computer skills (Microsoft Windows)

Responsibilities

• Assist the Physical Therapist in planning and implementing patient care and therapy services
• Assist patients with exercises, mobility, and activities of daily living (ADLs) as directed by the PT
• Educate patients and caregivers on proper techniques to improve mobility, reduce pain, and prevent injuries
• Maintain accurate and timely documentation of patient progress
• Collaborate with other healthcare professionals to ensure patients receive comprehensive care
• Provide home health and/or outpatient services as needed to ensure optimal outcomes
