Job summary

Discovery at Home is hiring a Physical Therapist to provide in-home services across Florida. The role emphasizes comprehensive assessments, creating and updating treatment plans with physician input, and using a range of modalities to reduce pain and improve function.

You will educate families, supervise staff, and ensure timely OASIS documentation in compliance with agency policies. The position supports patients in their own homes, coordinating care with physicians and the broader Discovery

Qualifications

• Must be a graduate of a Physical Therapy program approved by relevant medical bodies.
• Must be licensed or registered by the applicable state without restrictions.
• One year of home health experience preferred.
• Knowledge of Medicare and Medicaid guidelines.
• Valid driver's license and reliable transportation.
• Current CPR certification and good communication skills.

Responsibilities

• Provides initial and ongoing comprehensive assessments of client needs, including OASIS.
• Establishes and revises treatment plans with physician approval as needed.
• Treats clients to relieve pain and restore function using physical modalities.
• Teaches and counsels family and client in the physical therapy program.
• Instructs clients in use of mobility aids and devices.
• Supervises assistants and paraprofessionals monthly.
• Participates in case conferences and in-service programs.
• Completes and submits required OASIS documents within time frames.
