Job summary

Discovery at Home Gulf Coast is seeking a licensed Physical Therapist to provide comprehensive home health services in the Sun City, Florida area. You will assess patients, develop and adjust treatment plans with physician input, and supervise therapy aides as needed.

You should have a PT degree, valid FL license, CPR, driving license, and strong communication skills. The role includes flexible scheduling, competitive pay, and a supportive team within the Discovery Senior Living network.

Qualifications

• Must be a graduate of a Physical Therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Physical Therapy Association.
• Must be licensed or registered by the applicable State without restrictions.
• One year of home health experience required.
• Must have knowledge of Medicare and Medicaid guidelines
• Must have a valid driver’s license and reliable transportation.
• Must have a current CPR certification.
• Excellent verbal and written communication skills.
• Basic computer skill proficiency. Ability to use Microsoft Windows.

Responsibilities

• Provides initial and ongoing comprehensive assessments of the client’s needs, including Outcome and Assessment Information Set (OASIS) assessments.
• Establishes a treatment plan in consultation with the physician and revises it with the approval of the physician as necessary.
• Treats clients to relieve pain, develop or restore functional and maintain maximum performance, using physical means, such as exercise, massage, heat, water, light and electricity.
• Teaches, supervises, and counsels the family and client in the physical therapy program.
• Instructs client in care and use of wheelchairs, braces, crutches, canes and prosthetic and orthotic devices.
• Supervises Physical Therapy Assistants and paraprofessionals at least once monthly.
• Participates in case conferences, physical therapy staff meetings, agency activities and in-service programs as necessary.
• Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges, visit notes and significant change in condition in accordance with agency defined time frames.
