Qualifications

• Licensed physical therapist with current Florida license
• One year of home health experience required
• Knowledge of Medicare and Medicaid guidelines
• Valid driver’s license and reliable transportation
• Excellent verbal and written communication skills
• Basic computer skills including Microsoft Windows

Responsibilities

• Conduct initial evaluations to assess patients' needs and develop individualized treatment plans
• Develop and implement treatment plans based on patient needs and goals
• Monitor patient progress and adjust treatment plans as necessary
• Educate patients and caregivers on techniques to improve mobility and reduce pain
• Maintain accurate documentation of patient progress
• Collaborate with other healthcare professionals to ensure comprehensive care
• Provide home health and/or outpatient services, including supervisory visits, as needed
