Family Life Care, Inc. is seeking dedicated Physical Therapists to join our team for compassionate in-home care across our community in Florida. You will manage patient care from assessment through treatment, coordinating with physicians and care managers to achieve optimal outcomes.

Qualifications

• Current Florida Physical Therapy license.
• Current CPR Certification.
• Driver's License and Vehicle Insurance.
• Current TB test with negative results.
• Ability to pass a Level II background check.

Responsibilities

• Patient assessment, admission, and coordination of care with other disciplines, physicians and care managers.
• Provide Physical Therapy and treatment of home care patients in accordance with the plan of treatment and with company policies and procedures.
• Provide Physical Therapy in a pleasant, professional manner to maximize positive outcomes and patient satisfaction.
• Ensure that all clinical assessments, care plans, starting and resumption of care are completed with agency guidelines. Must be deadline driven.
• Train, Coach and Direct clinical staff in the performance of their clinical duties.
• Responsible for compliance with all state and federal, laws, regulations, policies, and processes relevant to home health.
