Job summary

UnitedHealth Group through Deaconess HomeCare (LHC Group) invites you to join as a Physical Therapist in Home Health. You will perform assessments and evaluations of patient care needs related to functional status and mobility, develop a treatment plan, provide interventions, and evaluate progress.

This PRN or full-time role offers a comprehensive benefits package, two compensation streams (per-visit and hourly), and a supportive team focused on caring, connecting and growing together.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsibilities

• Evaluates the patient's functional status and needs; assists in plan of care.
• Provides PT services within scope of practice per plan of care.
• Observes and reports patient progress to supervisor or physician.
• Treats patients to relieve pain, restore function, and maintain maximum functioning.
• Instructs patient and family about plan of care and progress toward goals.
• Completes evaluations and develops the PT plan of care per state guidelines.
