Qualifications

• Current Physical Therapy licensure in state of practice.
• Valid driver’s license and access to reliable transportation.
• CPR certification or ability to obtain within 90 days of hire.

Responsibilities

• Evaluate patient's functional status and therapy needs; assist in plan of care.
• Provide PT services within scope of practice and plan of care.
• Observe and report patient response to treatment and changes to care team.
• Treat patients to relieve pain, restore function, and maximize functioning.
• Instruct patient and family on plan of care, limitations and progress.
• Visit patients according to plan; complete progress notes per policy.
• Develop PT plan of care within state guidelines.
• Report outcomes and anticipated frequency of care.
• Function in diverse home settings regardless of background.
