Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Preferred CPR certification within 90 days of hire (optional)

Responsibilities

• Evaluates patient functional status and PT needs; assists in developing a therapy plan.
• Provides PT services within scope of practice and plan of care.
• Observes and reports patient response to treatment to nurse/physician; documents progress notes.
