Job summary

UnitedHealth Group is seeking a Physical Therapist in Home Health to assess patient needs, determine treatment plans, and provide interventions to improve mobility, endurance, and well-being. You will collaborate with physicians, nurses, and caregivers to ensure a cohesive plan of care and monitor patient progress.

This role emphasizes direct patient care in home settings, with responsibilities for visits, documentation, and coordination with the health care team.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Preferred CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs.
• Provides services within the scope of practice as defined by state laws and the plan of care.
• Observes and reports patient response to treatment to supervising nurse or physician.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning.
• Instructs patient and family/caregivers regarding the plan of care and progress toward goals.
• Visits patients according to Plan of Treatment and submits progress notes per policy.
• Completes PT evaluations and develops the plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and anticipated frequency of care.
