Job summary

UnitedHealth Group is seeking a Physical Therapist for home health, conducting assessments for functional status, mobility, endurance, and pain. You will develop and adjust treatment plans, perform interventions, and evaluate patient progress in collaboration with the health care team.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current CPR certification.
• Valid driver’s license and access to a dependable vehicle or public transportation.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition.

Responsibilities

• Evaluates the patient’s functional status and PT needs; assists physician in plan of care.
• Provides PT services within the scope of practice and plan of care.
• Observes and reports patient response to treatment to supervising nurse/physician.
• Treats to relieve pain, restore function, and maintain maximum functioning.
• Instructs patient and family on plan of care and progress toward goals.
• Visits patients per Plan of Treatment and documents progress notes.
