Job summary

UnitedHealth Group is seeking a Full Time Physical Therapist in a home health setting in Alabama. You will evaluate patient needs, develop and implement treatment plans, and coordinate with a broader care team to improve wellbeing and functional status.

The role emphasizes adherence to state guidelines and comprehensive documentation of progress. The position offers a mix of visit-point and hourly compensation, with a comprehensive benefits package and opportunities for professional growth

Qualifications

Current Physical Therapy licensure in state of Alabama.
Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
Current CPR certification or ability to complete within 90 days of hire.

Responsibilities

Evaluates the patient's functional status and physical therapy needs; assists in developing the plan of care.
Provides services within the scope of practice and in coordination with the health care team.
Observes and reports patient response to treatment and changes in condition to supervising staff.
Treats patients to relieve pain, improve function, and maintain maximum functioning.
Instructs patients and family on plan of care, limitations, and progress toward goals.
Visits patients per Plan of Treatment; completes progress notes per policy.
Completes evaluations and develops PT plan of care within state guidelines.
Reports outcomes and anticipated frequency of care.
