You will assess mobility and function, develop personalized care plans, and document progress for physician orders. The role emphasizes coordination with families and healthcare teams, supervision of support staff, and adherence to regulatory requirements.

Qualifications

• PT degree from an accredited program.
• Active PT license in Iowa.
• CPR certification.
• Experience in home health and Oasis documentation preferred.

Responsibilities

• Conduct initial and ongoing assessments of mobility, strength, coordination, and functional status.
• Administer, evaluate, and interpret tests to set goals and measure outcomes.
• Develop personalized plans of care and document progress, adjusting treatments as needed.
• Deliver therapy services aligned with physician orders and ensure compliance.
• Participate in discharge planning and educate patients and caregivers on home exercise programs.
• Coordinate with healthcare teams, physicians, and families, maintaining clear communication.
• Document accurately to prevent claim denials and comply with policies and regulations.
• Supervise PTAs and HHAs as applicable, ensuring quality care.
• Complete OASIS assessments where permitted by regulation.
