Job summary

Adaptive Home Health, LLC is seeking a Physical Therapist Assistant (PTA) to deliver skilled visits in patients' homes under a supervising PT. This field-based role focuses on functional mobility, gait training, transfers, and home safety to drive measurable outcomes.

You will be supported by an experienced clinical and operations team, work within a productivity model based on visits, and benefit from scheduling coordination and our charting tools designed to reduce documentation burden.

Qualifications

• Active Texas PTA license in good standing.
• Strong clinical judgment and ability to work independently in the field under PT supervision.
• Clear documentation and communication of patient status and changes.
• Reliable follow-through on treatment plans and goals.
• Willingness to travel daily across assigned territory.
• Carry and transport basic therapy equipment as needed.
• Competency in core home health PTA scope.
• Safety-focused mobility skills: transfers, gait, balance, fall prevention.
• Strengthening, functional mobility, and home safety assessment.

Responsibilities

• Deliver skilled PTA visits in the home under the supervising PT’s plan of care.
• Execute treatment plans focused on functional mobility and progress.
• Set/track objective progress and communicate updates to the supervising PT.
• Drive multidisciplinary coordination to keep the care team moving.
• Coach patients and caregivers with practical home programs.
• Document promptly and thoroughly in Homecare Homebase (HCHB).
