Job summary

Loving-Care-in-Home-Health-Services is seeking a licensed Physical Therapist to provide home health PT services under physician-prescribed treatment plans. You will evaluate patients, design plans of care, and guide families in home-based exercises and daily living activities.

Responsibilities include discharge planning, care coordination with physicians and staff, and timely documentation in WellSky. Medicare home health experience is preferred for this role.

Qualifications

• Minimum of 1 year of clinical and/or professional experience as a licensed Physical Therapist.
• Medicare Home Health experience is preferred.
• WellSky EMR software experience is preferred.

Responsibilities

• Perform initial patient evaluations and provide routine Physical Therapy services.
• Assess and treat patients using prescribed plans of treatment.
• Develop patient-specific treatment plans.
• Instruct patients and families in home-based care and activities of daily living.
• Initiate discharge planning when appropriate.
• Participate in weekly patient case conference meetings and staff meetings.
• Communicate with office staff, physicians and other therapists.
• Complete all evaluation documentation within 48 hours of evaluation.
• Complete care visit notes within 24 hours of visits.
• Utilize WellSky for EMR documentation.
