Job summary

Recover-Care seeks a Physical Therapist to develop individualized therapy programs, assess patients, and plan and implement treatment to maximize functional goals. You will educate patients and coordinate with other health professionals to support progress.

You will deliver care in SNF settings, document evaluations and progress, and stay current with professional standards and quality improvement activities. A state license is required and travel may be needed.

Qualifications

• Graduate of an accredited physical therapy program (Bachelors, Masters, or Doctorate).
• Current license from the state Board of Physical Therapy Examiners.
• Proficiency with applicable state, federal, and professional standards.
• Ability to travel as needed to perform job.

Responsibilities

• Provides direct patient care, evaluates functional needs and outcomes, and consults with other specialists.
• Designs and implements a plan of care for patients based on assessment.
• Conducts therapy practice within defined standards of care.
• Directs the assistance of support staff to achieve patients’ functional goals.
• Communicates effectively with other professionals and staff to achieve positive outcomes.
• Documents evaluations, goals, and plans; maintains signatures.
• Keeps up to date on professional literature and participates in education.
