Job summary

Enhabit Home Health & Hospice is seeking a licensed Physical Therapist to deliver skilled services in patients' homes across our programs. You will perform comprehensive assessments of mobility and function, design and implement individualized plans of care, monitor progress, modify interventions as needed, and educate patients and families to promote safe, independent daily living.

Qualifications

• Must possess a valid state driver’s license.
• Must possess automobile liability insurance.
• Must wear seatbelts at all times while driving.
• Must have dependable transportation kept in good working condition.
• Must be able to drive an automobile in all types of weather conditions.
• Must be currently licensed in the state of employment.

Responsibilities

• Examine and treat patients to improve mobility and self-care.
• Deliver skilled PT services under physician orders and plan of care.
• Teach families and care teams to support ongoing home-based therapy.
• Coordinate with the Branch Director and other care team members.
