Job summary

Choice Health At Home in Athens, TX, is seeking a Home Health Physical Therapist (PT) to deliver patient-centered care in the home health setting. You will evaluate patients, plan individualized treatment, and coordinate with physicians, nurses, and social workers to promote independence and safe mobility.

The role requires successful completion of an accredited PT program and a valid state license, CPR certification, and a driver’s license.

Qualifications

• Successful completion of an accredited physical therapy program.
• Current state Physical Therapy license.
• CPR certification is required.
• Valid driver’s license is required.
• 1 year of home health experience preferred.

Responsibilities

• Perform and document an initial exam, evaluating data to identify problems and determine a diagnosis prior to intervention.
• Plan, prepare, or carry out individually designed programs of physical treatment to maintain, improve, or restore physical functioning.
• Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.
• Applies concepts of infection control/universal precautions in performing patient care.
• Assure continuity of quality patient care delivered with appropriate documentation.
• Provide effective communication to patient/family, team members and other health care professionals.
• Promote agency philosophy and administrative policies.
• Maintain and complete all documentation within company stated guidelines.
