Job summary

VitalCaring is seeking a Home Health Physical Therapist to deliver patient-centered care in home settings. You will evaluate patients, develop individualized plans, and provide hands-on therapy to restore mobility and independence.

You will educate patients and families, monitor progress, and coordinate with an interdisciplinary team. The role offers autonomy, opportunities for case collaboration, and advancement within home health services.

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or Master’s).
• Current PT license in the state of practice.
• Minimum 1–2 years of experience (home health experience preferred).
• Strong clinical assessment and critical thinking skills.
• Excellent communication and patient education abilities.
• Current CPR certification.

Responsibilities

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential.
• Develop and implement individualized care plans in collaboration with physicians and the care team.
• Provide hands-on therapy to improve mobility, strength, balance, and independence.
• Educate patients and caregivers on treatment plans, exercises, and safety strategies.
• Monitor patient progress and adjust treatment plans as needed.
• Support safe transitions of care and contribute to discharge planning.
• Identify additional patient needs and collaborate with interdisciplinary teams.
• Partner with nurses, therapists, and physicians to ensure coordinated care.
• Document patient care and progress accurately and timely within the EMR.
• Participate in case conferences and team meetings.
• Supervise Physical Therapist Assistants (PTAs) in accordance with state guidelines.
• Provide guidance and education to caregivers and support staff.
