Job summary

VitalCaring Group is seeking a Home Health Physical Therapist to evaluate patients at home, develop individualized treatment plans, and provide hands-on therapy to improve mobility and independence. You will educate patients and caregivers, monitor progress, and coordinate with a multidisciplinary team to ensure safe transitions of care.

The role emphasizes autonomy, compassionate care, and ongoing clinical education while delivering high-quality home-based services across communities.

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or Master’s).
• Current PT license in the state of practice.
• Minimum 1–2 years of home health experience preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and patient education abilities.
• Current CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Evaluate and assess patients’ functional status, mobility, and rehabilitation potential.
• Develop and implement individualized care plans with physicians and care team.
• Provide hands-on therapy to improve mobility, strength, balance, and independence.
• Educate patients and caregivers on treatment plans, exercises, and safety.
• Monitor progress and adjust treatment plans as needed.
• Support transitions of care and participate in discharge planning.
• Document patient care and progress in the EMR.
• Collaborate with nurses, therapists, and physicians for coordinated care.
