Job summary

VitalCaring Group is seeking a Home Health Physical Therapist (PT) to deliver patient-centered care in patients' homes. You will evaluate mobility, create individualized plans with the care team, provide hands-on therapy, and educate patients and families to support independence.

The role emphasizes autonomy, collaboration with nurses and physicians, and continuous documentation. A PT license in the state, CPR, and a valid driver’s license are required, with 1–2 years of home health experience

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or Master’s).
• Current PT license in the state of practice.
• Minimum 1-2 years of experience (home health experience preferred).
• Strong clinical assessment and critical thinking skills.
• Excellent communication and patient education abilities.
• Current CPR certification.
• Valid driver's license, reliable transportation, and auto insurance.

Responsibilities

• Deliver patient-centered care: evaluate functional status, develop and implement individualized care plans with physicians and team, provide hands-on therapy to improve mobility, educate patients and caregivers on treatment plans, exercises, and safety strategies.
• Drive outcomes & recovery: monitor progress, adjust plans, support safe transitions of care and discharge planning, identify needs and collaborate with interdisciplinary teams, document care accurately in EMR, participate in case conferences.
• Lead & support: supervise PTAs per state guidelines, provide guidance to caregivers and support staff.
