Job summary

VitalCaring Group in Beaumont, TX is seeking a Physical Therapist (PT) to deliver patient-centered home health services. This full-time role offers autonomy, clinical growth, and meaningful outcomes in patients’ homes.

You will evaluate functional status, develop care plans with physicians, educate patients and caregivers, supervise PTAs, and document progress in the EMR. Requires DPT or Master’s, TX PT license, 1–2 years home health experience, CPR, and a valid driver’s license.

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or Master’s).
• Current PT license in the state of practice.
• Minimum 1–2 years of home health experience preferred.
• Strong clinical assessment and communication skills.
• CPR certification and valid driver’s license.

Responsibilities

• Evaluate patients’ functional status, mobility, and rehabilitation potential.
• Develop and implement individualized home health care plans.
• Provide hands-on therapy to improve mobility, strength, balance.
• Educate patients and caregivers on treatment plans and exercises.
• Monitor progress and adjust plans; participate in discharge planning.
• Document care accurately in the EMR and participate in team meetings.
