Job summary

VitalCaring Group in Hutchinson, Kansas seeks a dedicated Physical Therapist to join our home health team. You will assess patients in their homes, design personalized treatment plans, and help patients regain mobility and independence.

This is a highly autonomous, patient-centered role with collaboration from nurses and physicians. Requirements include a DPT or Master's in Physical Therapy, current KS license, 1–2 years home health experience, CPR, and a valid driver's license.

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or Master’s)
• Current PT license in the state of practice (KS)
• Minimum 1–2 years of home health experience preferred
• Strong clinical assessment and critical thinking skills
• Excellent communication and patient education abilities
• Current CPR certification
• Valid driver’s license and reliable transportation

Responsibilities

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential
• Develop and implement individualized care plans in collaboration with physicians and the care team
• Provide hands‑on therapy to improve mobility, strength, balance, and independence
• Educate patients and caregivers on treatment plans, exercises, and safety strategies
• Monitor patient progress and adjust treatment plans as needed
• Support safe transitions of care and contribute to discharge planning
• Document patient care and progress accurately and timely within the EMR
• Participate in case conferences and team meetings
• Supervise Physical Therapist Assistants (PTAs) in accordance with state guidelines
• Provide guidance and education to caregivers and support staff
