VitalCaring Group is seeking a Physical Therapist (PT) for Home Health on a PRN basis in Louisiana. You will perform patient-centered home visits, develop individualized plans, and collaborate with a care team to improve mobility and independence.

Responsibilities include evaluating status, educating patients and caregivers, supervising PTAs per state guidelines, and documenting progress in the EMR. A DPT or MS, active PT license, CPR, and a valid driver's license are required.

Qualifications

• Graduate from an accredited PT program (DPT or MS).
• Current PT license in the practicing state.
• 1–2 years of home health experience preferred.
• CPR certification and valid driver’s license.

Responsabilités

• Evaluate patients’ functional status, mobility, strength, and rehabilitation potential.
• Develop and implement individualized care plans with physicians and care team.
• Provide hands-on therapy to improve mobility, strength, balance, and independence.
• Educate patients and caregivers on treatment plans and safety strategies.
• Monitor patient progress and adjust treatment plans as needed.
• Support safe transitions of care and contribute to discharge planning.
• Identify additional patient needs and collaborate with interdisciplinary teams.
• Partner with nurses, therapists, and physicians for coordinated care.
• Document patient care and progress accurately within the EMR.
• Participate in case conferences and team meetings.
• Supervise Physical Therapist Assistants (PTAs) following state guidelines.
