Job summary

VitalCaring is seeking a Home Health Physical Therapist (PT) in Oklahoma to deliver patient-centered care in home settings. You will assess functional status, develop plans, and provide hands-on therapy to improve mobility and independence.

The role emphasizes autonomy, collaboration with physicians and nurses, and supporting discharge planning while maintaining exceptional patient care and education for patients and families.

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or Masters).
• Current PT license in the state of practice.
• Minimum 19 years of home health experience (preferred).
• CPR certification.
• Valid drivers license and reliable transportation.

Responsibilities

• Evaluate and assess patientsunctional status, mobility, strength, and rehabilitation potential.
• Develop and implement individualized care plans with physicians and the care team.
• Provide hands-on therapy to improve mobility, strength, balance, and independence.
• Educate patients and caregivers on treatment plans, exercises, and safety strategies.
• Monitor patient progress and adjust treatment plans as needed.
• Support safe transitions of care and contribute to discharge planning.
• Identify additional patient needs and collaborate with interdisciplinary teams.
• Partner with nurses, therapists, and physicians to ensure coordinated care.
• Document patient care and progress accurately in the EMR.
• Participate in case conferences and team meetings.
• Supervise Physical Therapist Assistants (PTAs) per state guidelines.
• Provide guidance and education to caregivers and support staff.
