Job summary

AHN Healthcare@Home is seeking a licensed Physical Therapist (PT) to provide skilled home health services in the Suttersville and West Newton, PA area. A $10,000 first-year success bonus is offered and grows with ongoing performance.

You will collaborate with an interdisciplinary team, travel to patient homes, and contribute to patient recovery and safety at home. Benefits include medical/dental/vision, fleet vehicle program, generous PTO, 401K with company match, paid parental leave, and Purdue

Qualifications

• Active Physical Therapist license in state you work
• One year of experience as a Physical Therapist
• Ability to drive to patient locations ~20% of workday

Responsibilities

• Deliver skilled, patient-centered care in a home health setting with interdisciplinary team support
• Perform comprehensive patient evaluations and develop therapy plans to improve strength, balance, and independence
• Educate patients and caregivers on exercises, injury prevention, and safe mobility at home
• Ensure compliant EMR documentation and proper charting
• Work independently while collaborating with the team
• Travel to patient homes within assigned territory

• Educate and empower patients and caregivers on therapeutic exercises, injury prevention, and safe mobility strategies to support recovery and confidence at home
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation

PT Qualifications

• Active Physical Therapist, PT license in the state you work
• One year of experience as a Physical Therapist and the clinical expertise to deliver quality patient care
• Due to the field-based nature of this home health position, a valid driver's license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
