Job summary

AHN Healthcare@Home is seeking a Physical Therapist (PT) to provide skilled home health services in the Waynesburg, Prosperity, and Amity, PA area. This role includes a $10,000 first-year success bonus and opportunities for ongoing rewards as you grow with us.

You will evaluate patients, develop individualized therapy plans, educate families, and document care using EMR systems in a supportive interdisciplinary team environment.

Qualifications

• Active Physical Therapist license in the state you work
• One year of experience as a Physical Therapist
• Valid driver’s license and ability to spend approximately 20% of your workday driving to patient locations

What You’ll Do:

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• Perform comprehensive patient evaluations, functional mobility assessments, and develop individualized therapy plans to improve strength, balance, and independence
• Educate and empower patients and caregivers on therapeutic exercises, injury prevention, and safe mobility strategies to support recovery and confidence at home
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation

• Work independently, but never alone
• Travel to patient homes within assigned territory

PT Qualifications

• Active Physical Therapist, PT license in the state you work
• One year of experience as a Physical Therapist and the clinical expertise to deliver quality patient care
• Due to the field-based nature of this home health position, a valid driver's license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
