Residential Home Health and Hospice in Mount Carmel, PA, is hiring a Physical Therapist to deliver skilled care in patients’ homes.

This field-based role includes travel within the assigned territory, a dedicated support team, and a first-year $10,000 success bonus.

Qualifications

• Active Physical Therapist license in the state you work
• One year of PT experience delivering patient care
• Valid driver’s license and ability to travel ~20% of workday

What You’ll Do:

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• Perform comprehensive patient evaluations, functional mobility assessments, and develop individualized therapy plans to improve strength, balance, and independence
• Educate and empower patients and caregivers on therapeutic exercises, injury prevention, and safe mobility strategies to support recovery and confidence at home
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation

• Work independently, but never alone
• Travel to patient homes within assigned territory

PT Qualifications

• Active Physical Therapist, PT license in the state you work
• One year of experience as a Physical Therapist and the clinical expertise to deliver quality patient care
• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
