Graham Healthcare Group in Sunbury, PA is seeking a dedicated Physical Therapist (PT) for home health. This full-time role delivers skilled care to patients in their homes, guided by an interdisciplinary team, with strong support for your growth.You will assess and treat, educate patients and caregivers, document in EMR, and travel within the assigned territory. Active PT license in PA, at least 1 year of PT experience, and a valid driver's license required.

Qualifications

• Active Physical Therapist license in the state you work (PA).
• Minimum of 1 year of PT experience delivering quality patient care.
• Current driver’s license and ability to travel to patient homes (~20%).

Responsibilities

• Deliver skilled, patient-centered care in a home health setting with an interdisciplinary team.
• Perform comprehensive evaluations, mobility assessments, and develop therapy plans.
• Educate patients and caregivers on exercises, safety, and recovery strategies.
• Document thoroughly in the EMR and ensure compliance with regulations.
• Work independently while traveling to patient homes within the territory.

What You’ll Do:

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• Perform comprehensive patient evaluations, functional mobility assessments, and develop individualized therapy plans to improve strength, balance, and independence
• Educate and empower patients and caregivers on therapeutic exercises, injury prevention, and safe mobility strategies to support recovery and confidence at home
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation

• Work independently, but never alone
• Travel to patient homes within assigned territory

PT Qualifications

• Active Physical Therapist, PT license in the state you work
• One year of experience as a Physical Therapist and the clinical expertise to deliver quality patient care
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
