Job summary

AHN Healthcare@Home in Uniontown, PA is seeking a dedicated Physical Therapist to provide skilled, patient-centered home health care in patients’ homes.

You will evaluate function, develop individualized therapy plans, and educate patients and caregivers while delivering therapy and documenting care in the EMR. Travel to patient homes within your territory is required.

Qualifications

• Active Physical Therapist with a valid PT license in the state you work.
• One year of experience as a Physical Therapist delivering patient care.
• Driver's license required and ability to drive to patient locations (about 20% of day).

Responsibilities

• Deliver skilled, patient-centered home health care in collaboration with an interdisciplinary team.
• Perform comprehensive patient evaluations and develop therapy plans to improve strength, balance, and independence.
• Educate patients and caregivers on exercises, injury prevention, and safe mobility at home.
• Document care thoroughly in the EMR and ensure compliance.
• Travel to patient homes within your assigned territory.

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• Perform comprehensive patient evaluations, functional mobility assessments, and develop individualized therapy plans to improve strength, balance, and independence
• Educate and empower patients and caregivers on therapeutic exercises, injury prevention, and safe mobility strategies to support recovery and confidence at home
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation

PT Qualifications

• Active Physical Therapist, PT license in the state you work
• One year of experience as a Physical Therapist and the clinical expertise to deliver quality patient care
• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
