Residential Home Health and Hospice in Palmyra, PA seeks a licensed Physical Therapist for home health care across the assigned territory. You will deliver skilled therapy with an interdisciplinary team, evaluating patients and creating plans to improve mobility and independence.

What You’ll Do:

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you

• Perform comprehensive patient evaluations, functional mobility assessments, and develop individualized therapy plans to improve strength, balance, and independence

• Educate and empower patients and caregivers on therapeutic exercises, injury prevention, and safe mobility strategies to support recovery and confidence at home

• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation

• Work independently, but never aloneTravel to patient homes within assigned territory

PT Qualifications

• Active Physical Therapist, PT license in the state you work

• One year of experience as a Physical Therapist and the clinical expertise to deliver quality patient care

• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
