Job summary

People Well Staffing is seeking an experienced Physical Therapist (PT) for a permanent Home Health opportunity serving the West Valley of Phoenix, Arizona. You will evaluate patients in their homes, develop plans of care, and provide gait, balance, and functional training to improve independence.

The position offers a $140,000 annual salary, a $2,000 sign-on bonus, mileage reimbursement, and the autonomy of a home-based caseload with an interdisciplinary team.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Active Arizona Physical Therapist license in good standing.
• Home Health experience required.
• Current BLS/CPR certification.
• Valid driver's license and reliable transportation.
• Excellent communication, organization, time-management, and documentation skills.
• Strong clinical judgment and ability to work independently.

Responsibilities

• Perform comprehensive in-home Physical Therapy evaluations and reassessments.
• Develop and implement individualized plans of care.
• Provide therapeutic exercise, gait, balance, mobility, and functional training.
• Help patients improve safety and independence in their home environment.
• Educate patients and caregivers on home programs, fall prevention, and safety.
• Communicate patient progress and changes with physicians and interdisciplinary team members.
• Maintain accurate and timely clinical documentation.
• Effectively manage an assigned Home Health territory and caseload.
