AHN Healthcare@Home is hiring a Home Health Physical Therapist to deliver skilled care to patients in their homes in the Mount Pleasant area. A $10,000 first-year bonus is available for achieving key milestones.

You will evaluate patients, plan individualized therapy, educate families, and maintain compliant EMR documentation while traveling within the assigned territory., the role offers growth and supportive leadership.

Qualifications

• Active Physical Therapist license in the state you work.
• Minimum of 1 year of experience as a Physical Therapist delivering patient care.
• A valid driver’s license and ability to drive to patient locations ~20% of workday.

Responsibilities

• Deliver skilled, patient-centered care in home health settings with an interdisciplinary team.
• Perform comprehensive patient evaluations and develop individualized therapy plans.
• Educate patients and caregivers on therapeutic exercises and safe mobility.
• Ensure compliant EMR documentation and effective problem solving.
• Work independently yet stay collaborative with team.
• Travel to patient homes within the assigned territory.

What You’ll Do:

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• Perform comprehensive patient evaluations, functional mobility assessments, and develop individualized therapy plans to improve strength, balance, and independence
• Educate and empower patients and caregivers on therapeutic exercises, injury prevention, and safe mobility strategies to support recovery and confidence at home
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory

PT Qualifications

• Active Physical Therapist, PT license in the state you work
• One year of experience as a Physical Therapist and the clinical expertise to deliver quality patient care
• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
