Job summary

Residential Home Health and Hospice is seeking a Physical Therapist to provide skilled, patient-centered care in patients’ homes in the Chambersburg, PA area. The role offers meaningful autonomy with an interdisciplinary team and opportunities for growth.

Eligible for substantial bonus potential and a path for ongoing rewards. Must have PT license in state and be able to travel to patient locations in the assigned territory.

Qualifications

• Active Physical Therapist license in the state you work.
• One year of physical therapy experience delivering home health care.
• Valid driver’s license with ~20% daily driving to patient locations.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting with an interdisciplinary team.
• Perform comprehensive patient evaluations and develop individualized therapy plans.
• Educate patients and caregivers on exercises, mobility strategies, and safety.
• Think critically and document thoroughly in the EMR.
• Work independently, but collaborate with team as needed.
• Travel to patient homes within assigned territory.

We are hiring a Physical Therapist (PT) for a Home Health position providing skilled care to patients in their homes in the Chambersburg, PA area. This position is eligible for $20,000 total bonus potential, including a $10,000 first year success bonus to reward the accomplishment of key achievements during your first year as well as an additional $10,000 sign-on bonus paid after 30 days of employment. To receive these bonuses, you must be in good standing & meet performance expectations, and remain in the role and status for which you were hired.

What You’ll Do:

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you.
• Perform comprehensive patient evaluations, functional mobility assessments, and develop individualized therapy plans to improve strength, balance, and independence.
• Educate and empower patients and caregivers on therapeutic exercises, injury prevention, and safe mobility strategies to support recovery and confidence at home.
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation.
• Work independently, but never alone.
• Travel to patient homes within assigned territory.

PT Qualifications

• Active Physical Therapist, PT license in the state you work.
• One year of experience as a Physical Therapist and the clinical expertise to deliver quality patient care.
