Job summary

Residential Home Health and Hospice is seeking a full-time Physical Therapist to provide skilled home health care in the Lehighton, PA area. A $10,000 first-year success bonus rewards key achievements during the initial year.

You will rotate through visits with an interdisciplinary team, educate patients and caregivers, and document care in the EMR. A PT license in the state and driving capability are required.

Qualifications

• Active Physical Therapist license in the state you work.
• At least 1 year of PT experience delivering patient care.
• Driver's license and ability to travel ~20% of the day.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting with an interdisciplinary team.
• Perform comprehensive evaluations and develop therapy plans to improve strength, balance and independence.
• Educate patients and caregivers on exercises, injury prevention, and safe mobility at home.
• Think critically and ensure thorough EMR documentation.
• Work independently, but stay connected with the team and patients.
• Travel to patient homes within the assigned territory.

What You'll Do:

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• Perform comprehensive patient evaluations, functional mobility assessments, and develop individualized therapy plans to improve strength, balance, and independence
• Educate and empower patients and caregivers on therapeutic exercises, injury prevention, and safe mobility strategies to support recovery and confidence at home
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory

PT Qualifications

• Active Physical Therapist, PT license in the state you work
• One year of experience as a Physical Therapist and the clinical expertise to deliver quality patient care
• Current driver's license and ability to spend ~20% of your day driving to/from patient locations
