Job summary

AHN Healthcare@Home is hiring a licensed Physical Therapist to provide skilled, patient-centered care in patients' homes across Greensburg and surrounding areas.

In this role you will perform thorough evaluations, design individualized therapy plans, educate patients and caregivers, and document care in an EMR system. You will travel to patient homes and collaborate with an interdisciplinary team to support independence and recovery.

Qualifications

• Active Physical Therapist license in your practicing state.
• Minimum 1 year of clinical PT experience delivering patient care.
• A valid driver’s license and the ability to drive approximately 20% of the workday.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting in collaboration with an interdisciplinary team.
• Perform comprehensive patient evaluations and develop individualized therapy plans to improve strength, balance, and independence.
• Educate patients and caregivers on therapeutic exercises, injury prevention, and safe mobility strategies.

PT Qualifications

• Active Physical Therapist, PT license in the state you work
• One year of experience as a Physical Therapist and the clinical expertise to deliver quality patient care
• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
