Residential Home Health and Residential Hospice is seeking a Home Health Physical Therapist to provide skilled care in patients' homes in the Metropolis, IL area. This role offers the opportunity for meaningful patient relationships, autonomy, and clear career growth.

Qualifications

• Active Physical Therapist license in the state you will work
• One year of experience as a Physical Therapist delivering quality patient care
• Current driver’s license and ability to drive ~20% of day to patient homes

Responsibilities

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that supports you
• Perform comprehensive patient evaluations, functional mobility assessments, and develop individualized therapy plans to improve strength, balance, and independence
• Educate and empower patients and caregivers on therapeutic exercises, injury prevention, and safe mobility strategies to support recovery and confidence at home
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory

PT Qualifications

• Active Physical Therapist, PT license in the state you work
