Job summary

Residential Home Health and Hospice is seeking a Home Health Physical Therapist to provide skilled care to patients in the East Kansas City area. This role offers autonomy, collaborative support, and meaningful patient relationships.

Eligible for a $10,000 first-year success bonus based on achieving key milestones. Travel to patient homes within the assigned territory is required, with strong growth opportunities and a supportive leadership team.

Qualifications

  • Active Physical Therapist license in the state you work.
  • One year of Physical Therapist experience.
  • Current driver’s license and ability to spend ~20% of your day driving to/from patient locations.

  • Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
  • Perform comprehensive patient evaluations, functional mobility assessments, and develop individualized therapy plans to improve strength, balance, and independence
  • Educate and empower patients and caregivers on therapeutic exercises, injury prevention, and safe mobility strategies to support recovery and confidence at home
  • Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
  • Work independently, but never alone
  • Travel to patient homes within assigned territory

PT Qualifications


  • Active Physical Therapist, PT license in the state you work
