Residential Home Health and Hospice is seeking a Physical Therapist to provide skilled care to patients in the Philadelphia, PA area. The position offers a first-year bonus and extensive growth opportunities while supporting autonomy and meaningful patient relationships.

You will travel to patient homes within the assigned territory and collaborate with an interdisciplinary team to deliver high-quality therapy and ensure compliant documentation.

Qualifications

• Active Physical Therapist license in the state you work.
• Minimum one year of experience as a Physical Therapist.
• A valid driver’s license and the ability to spend ~20% of your workday driving to patient locations.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you.
• Perform comprehensive patient evaluations, functional mobility assessments, and develop individualized therapy plans to improve strength, balance, and independence.
• Educate and empower patients and caregivers on therapeutic exercises, injury prevention, and safe mobility strategies to support recovery and confidence at home.
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation.
• Work independently, but never alone.
• Travel to patient homes within assigned territory.

PT Qualifications

• Active Physical Therapist, PT license in the state you work
• One year of experience as a Physical Therapist and the clinical expertise to deliver quality patient care
• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
