Job summary

Residential Home Health and Residential Hospice in Aurora, IL is seeking a Home Health Physical Therapist (PT) to provide skilled care to patients in their homes. This role offers a $20,000 total bonus potential, including a $10,000 first-year success bonus and a $10,000 sign-on paid within 18 months.

Expect a 40-hour work week with an autonomous yet collaborative practice, strong patient relationships, and room for professional growth within a nationally recognized home health leader.

Qualifications

• Active Physical Therapist license in Illinois is required.
• One year of Physical Therapist experience preferred.
• Current driver’s license and ability to drive ~20% of day.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting with an interdisciplinary team.
• Perform comprehensive patient evaluations, mobility assessments, and develop therapy plans to improve strength, balance and independence.
• Educate patients and caregivers on exercises, injury prevention, and safe mobility strategies.
• Think critically, ensure compliant EMR documentation.
• Work independently, but supported by the team.
• Travel to patient homes within assigned territory.

PT Qualifications

• Active Physical Therapist, PT license in the state you work
• One year of experience as a Physical Therapist and the clinical expertise to deliver quality patient care
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
