Job summary

Residential Home Health and Hospice in Michigan is seeking a Physical Therapist to provide skilled care to patients in their homes in the St Johns area. The position offers up to $20,000 total bonus potential, including a $10,000 first year success bonus and an additional $10,000 sign-on bonus.

Qualifications

• Active Physical Therapist license in the state you work.
• One year of experience as a Physical Therapist and the clinical expertise to deliver quality patient care.
• A valid driver's license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you.
• Perform comprehensive patient evaluations, functional mobility assessments, and develop individualized therapy plans to improve strength, balance, and independence.
• Educate and empower patients and caregivers on therapeutic exercises, injury prevention, and safe mobility strategies to support recovery and confidence at home.
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation.
• Work independently, but never alone.
• Travel to patient homes within assigned territory.
